1. What checking the consent box means

By filling out and submitting the Contact form or the Medical Referrals form, and checking the corresponding box, you are giving your express and informed consent for SAH to process the personal data — and, in the case of a referral, the health data — that you voluntarily included, for the sole purpose of responding to your inquiry or managing the referral.

2. Voluntary nature

Filling out these forms is voluntary. Fields marked as required are the minimum necessary for us to respond to you or process the referral; if you don't fill them out, we may not be able to process your request.

3. Withdrawing your consent

You can withdraw your consent at any time by writing to us at [PENDING INFORMATION — contact email]. This does not affect the lawfulness of processing carried out before the withdrawal.
